Serving Greater Cincinnati, Dayton, and Butler County.

Recovery isn’t one-size-fits-all. Our Intensive Outpatient Program brings psychiatry, therapy, and addiction treatment together in one coordinated program — giving you the structured support you need while you maintain work, family, and daily responsibilities.

About our IOP program

Our IOP serves adults (18+) who need structured treatment for substance use but don’t require 24-hour residential care. Whether you’re stepping down from residential treatment, stepping up from weekly therapy, or starting recovery for the first time, our program provides comprehensive care that fits your life.

  • You’re transitioning from residential treatment, detox, or hospitalization
  • Weekly therapy isn’t providing enough structure or support
  • You’re ready for group-based recovery and motivated to stay sober
  • You need to balance treatment with work or family responsibilities
  • You’ve completed medical detox and are ready to focus on recovery
  • You need to fulfill a court mandated diversion program requirement.

What's Included

Weekly Individual Counseling

50-minute one-on-one sessions with your dedicated counselor

Treatment for co-occurring disorders: depression , anxiety , PTSD, bipolar disorder, ADHD , OCD

Recovery does not happen in isolation, and neither does our care. Beyond clinical treatment, your care coordinator helps address the practical barriers that affect recovery — housing, transportation, benefits, and community resources — and connects you with the right support.

Relapse prevention is not a module at the end of the program — it is a thread running through every session from the beginning. Patients develop coping strategies, emotional regulation skills, and the self-awareness to recognize early warning signs before they escalate.

Crisis Resources

If you are experiencing a crisis outside of scheduled sessions, call or text 988 (Suicide & Crisis Lifeline) or call 911 for emergencies.

Court-Ordered Treatment or a Diversion Program Requirement?

If you have been ordered to complete an Intensive Outpatient Program as part of a diversion agreement, probation condition, or court-mandated treatment plan, SOS Wellness Virtual IOP is built to meet those requirements. We are CARF-accredited and licensed by OhioMHAS — the credentials Ohio courts and diversion programs recognize.

For courts, TASC officers & probation departments

  • CARF-accredited and OhioMHAS-licensed — meeting the clinical and regulatory standards Ohio courts require for IOP completion.
  • Attendance documentation provided — formal records maintained throughout the program and available for court reporting.
  • Certificate of completion issued — submittable directly to your attorney, TASC officer, or probation department.
  • Direct court coordination — we work with court monitors and diversion coordinators when a signed release is in place.
  • Drug screening coordinated locally — we coordinate with local lab partners, including LabCorp, for chain-of-custody screening.

Getting Started

Our clinical team uses treatments supported by research and clinical evidence:

Step 1: Request a Call

  • Complete the form on this page or call (513) 928-3339 (7AM–11PM). A member of our intake team will call to get you registered, assist you in setting up your MYIO patient portal, and schedule your first appointment with a therapist.

Step 2: Clinical Assessment

  • Your therapist will conduct a clinical assessment to confirm IOP is the right level of care and begin building your individualized treatment plan.

Step 3: Insurance Verification & Care Coordination

  • We verify your insurance and confirm coverage at no cost before your first session. You will also be scheduled with a care coordinator to assess any needs or barriers and connect you with appropriate community resources.

Step 4: Start the Program

  • Once your group is ready, your therapist will schedule your start date at our Middletown clinic.

Step 5: Ongoing Care

  • As you complete IOP, your clinical team transitions you into a structured step-down plan — with continued individual therapy, group check-ins, and ongoing medication management when needed. Recovery is supported well beyond the program.

Common Question

Outpatient care involves individual appointments (weekly, bi-weekly, or monthly) for medication management and therapy. IOP provides 9-12 hours of structured programming per week with group therapy, individual therapy, and medication management—ideal for those needing more intensive support while living at home.

We specialize in treating co-occurring disorders—it’s actually more effective to address both simultaneously. Our psychiatric team treats depression, anxiety, PTSD, and other mental health challenges alongside addiction recovery.

We help you transition to continuing care—whether that’s weekly therapy, support groups (AA, SMART Recovery), or ongoing psychiatric care. Your case manager ensures there’s no gap in support.

During your assessment, we’ll help determine the right level of care. IOP is typically best when weekly therapy isn’t providing enough structure or when stepping down from residential treatment.
