Respect and participation
You deserve to be treated with dignity and respect. Ask your care team about treatment options, expected benefits, possible risks, and alternatives. Your goals and preferences are part of planning your care.
Privacy and records
You have rights concerning your health information under applicable law. These may include access to records, requesting corrections, confidential communications, certain restrictions, and information about certain disclosures. The complete Notice of Privacy Practices explains the details and any applicable exceptions.
For current notices and the process for making a request, call 513-928-3339. Certain substance use disorder records have additional protections under 42 CFR Part 2.
Raise a concern
If you have concerns about your care or privacy, contact our office and ask for the grievance or privacy contact. Our original website identifies Kim Jarrell as Grievance Officer. You may raise concerns without fear of retaliation.
Call 513-928-3339, or write to SOS Wellness LLC, 6730 Roosevelt Ave, Suite 301, Middletown, OH 45005. Please call before sending sensitive information so the office can explain an appropriate secure channel.
